Student insurance plan

Overview

The CCAD student insurance plan provides a very convenient and cost-effective way for students to receive medical care while enrolled full-time at the college. All full-time CCAD students are automatically enrolled in the college’s student health insurance plan. Because mandatory participation allows the college to provide very low-cost health insurance coverage to all of our students, participation in this plan is required, even if additional insurance coverage is provided through other policies.

Benefits of the plan include:

  • No designated list of healthcare providers. Students can receive services from any doctor he/she chooses. However, using doctors from a list provided at www.beechstreet.com, or 800-432-1776 will help keep any out-of-pocket expenses lower should you exceed the maximum coverage amounts.
  • First dollar coverage for eligible expenses incurred for outpatient visit to a doctor or clinic (provided the costs stay within the maximum coverage amounts and the visit is not to an emergency room).
  • Costs for prescriptions are reimbursed for covered illnesses when combined with other miscellaneous charges not to exceed $500.00 (up to the maximum coverage amounts).
  • The college will verify the student’s enrollment to the insurance provider and the student only needs to show his/her insurance card to the healthcare provider to receive services, and submit a claim form to the insurance company (also available at the CCAD one-stop student services counter.)

Frequently Asked Questions:

What is a “covered sickness?”
Generally speaking, the plan covers any sickness (see the official insurance brochure for exclusions.) The plan does not cover dental work or eye care, unless the damage is a result of a specific injury. Elective surgery, preventive medicines or vaccines, and intentionally self-inflicted injuries are not covered either.

What is the limit of coverage for a general sickness when seen as an outpatient?
Eligible expenses are covered at 100% of usual and customary charges up to $500 per sickness. This includes charges by a radiologist or for laboratory tests.

What if I have to be admitted to a hospital (inpatient services?)
Eligible expenses are covered up at 100% of usual and customary charges, limited to a maximum payment of $1,000 per sickness as allocated in the schedule of benefits. (Example: semi-private room, $400 surgical allowance and $300 miscellaneous hospital charges.) Expenses incurred beyond $1,000 are eligible for payment of up to 80% of covered charges not to exceed $5,000 for any one covered sickness or injury.

Are emergency room visits covered?
Yes, but there is a $100 deductible for every visit to an ER. To avoid paying this deductible, you should try to be seen by a doctor at a regular clinic if at all possible. ER visits should only be for true emergencies.

Can you recommend a clinic or doctor?
A convenient clinic near campus is Capital Primary Care at 500 E. Main St. (corner of Main St. and Washington Ave. , just 5 blocks from campus.) They will usually make a same-day appointment for CCAD students. Simply call 614-566-9933 for an appointment and state that you are a CCAD student.

Are prescriptions covered?
Yes, prescriptions are eligible for coverage as part if the $500 miscellaneous outpatient benefit. You will first need to pay for the prescription yourself, and then submit proof of payment with a reimbursement request (claim form) to the insurance provider. These forms are also available at the CCAD student services counter, or by contacting Acordia at 1-800-228-6768 and on their website at www.acordia.com/colleges.

Are mental health issues covered?
Yes, but you must first be referred by a member of the CCAD counseling staff. An appointment can be made to see a college counselor through the CCAD studentservices counter.

Is there a maximum timeframe in which I must make a claim?
Yes, bills for which benefits are to be paid must be submitted within 90 days of treatment. You may continue to file claims for any one injury or illness for up to 52 weeks from the date of injury or the date of first treatment for a sickness.
