Transcript & enrollment verification requests

Official transcripts for all course work completed at CCAD and enrollment verifications are available from One Stop Student Services. At least 5–7 days must be allowed to process transcript and enrollment verification requests. Our institution has authorized the National Student Clearinghouse (NSC) to act as our agent for all verifications of student enrollment. The NSC also provides an online transcript ordering service for CCAD students and alumni.

Students requesting transcript and/or enrollment verification service should plan in advance. Last minute requests are difficult to process during certain periods of the academic year.

Submitting a request

Transcripts and enrollment verifications may be ordered in three ways—online, in person, or through the mail. Enrollment verifications are free of charge. The transcript fee is $5.00 per transcript for regular processing (5–7 days) and an additional $10 rush fee for rush processing (less than 2 days).

• Online: Current students and those with active CCAD IQ.Web accounts may access the National Student Clearinghouse Student Self-Service Site through IQ.Web to order transcripts and enrollment verifications online.
• Online transcript requests that are submitted while CCAD is closed for the holidays will be processed in the order they were received when One Stop Student Services reopens.
  
  Alumni and former students should use the secure clearinghouse website (link will open in a new window) to order transcripts online.
• In Person: Stop by One Stop Student Services on the first floor of the Loann Crane Center, Monday through Friday between 8:00 am and 5:00 pm.
• By Mail: Download, complete, and mail the Transcript Request Form (59K PDF) and/or Verification Request Form (54K PDF) to:

All requests must include the student's full name and maiden name, signature, Social Security number, date of birth, current address, years of attendance in day and evening classes, number of copies desired, and the complete names and addresses of individuals or organizations that are to receive the transcripts and/or enrollment verification.

• Complete the Transcript Request Form (59K PDF) to request copies of your official transcript for yourself or for other institutions.
• Complete the Verification Request Form (54K PDF) if you are requesting information be forwarded to another institution.

There is a $5 charge for each official transcript requested. There is an additional $10 rush fee for rush transcript requests (less than 2 days to process). In order to comply with the Family Educational Rights and Privacy Act of 1974, the College must have the student's written consent to release educational records. Enrollment verifications are free of charge.

Our institution is a member of the National Student Clearinghouse (NSC). NSC has been authorized to act as our agent for all verifications of student enrollment. NSC has now made it possible for students and companies needing current enrollment verifications to obtain instantaneous certifications of student enrollment on the NSC web site. Students can download or print this information, and forward it to any third party that needs it. This service is available 24 hours a day, 7 days a week, and is quickly becoming the preferred method of student verification.
